Today I met 20 years old child who has an aspirate pnumonia problem.Suddenly I met her I was very surprising because she apparently look like as 6 years girl more than 20 years old. After resusitation ,I screened her history from her family to ensure my hypothesis. From physical examination I found that she had failure to thrive , dry and pale skin , weight gain loss, protusive umbilical .Her history was very clear in hormone desease .For example ,she was extream fatigue after birth and she was usually cool sensitivity but her family did not concern about her problem and refused to treat her after that.

Diagnosis of hypothyroidism is based on symptoms and the results of blood tests that measure levels of TSH and sometimes the levels of the thyroid hormone thyroxine. Low levels of thyroxine and high levels of TSH indicate an underactive thyroid. So, the doctor want confirm his diagnosis by blood test .Unfortunately our lab have to wait for 2-3 days .so, I want to review knowlege about cretin child practically.

วันนี้ได้เจอเคสที่น่าสนใจเคสหนึ่งซึ่งมาด้วยอาการไข้และหายใจลำบาก ครั้งแรกที่พบคิดว่าเป็นเคสเด็ก ราวอายุ 6 ปี ปรากฏว่าพอซักอายุ พ่อผู้ป่วยให้ประวัติว่าอายุ 20 ปี ก็ประหลาดใจกันไปตามกัน หลังจากช่วยใส่ Tube และช่วยชีวิตแล้ว จึงได้มาไล่เลียงซักประวัติกันดู เนื่องจากพ่อเด็กบอกว่าเด็กสุขภาพดีไม่เคยเจ็บป่วยมาก่อนเลย ยิ่งทำให้เราประหลาดใจกันมาก เพราะจากที่ตรวจร่างกายจะพบว่า เด็กมีร่างกายที่อ่อนปวกเปียก พุงป่อง ลิ้นจุกปาก ผิวแห้งซีด และดูจะมีพัฒนาการไม่สมวัยอย่างชัดเจน นำห้นัก 20 kgs ได้ ซึ่งก็สันนิษฐานไว้ในใจว่าเข้าข่ายเด็ก cretin หรือเด็กเอ๋อ หลังจากซักประวัติทุกอย่างจึชัดเจนขึ้นไม่ว่าจะเป็นประวัติหลังคลอดซึ่งเด็กจะมีร่างกายอ่อนปวดเปียกมาตั้งแต่คลอด และเป็นคนขี้หนาว ซึ่งทางโรงพยาบาลจุฬาลงกรณ์ก็ได้แจ้งกับผู้ปกครองเด็กว่าเด็กมีความผิดปกติของฮอร์โมนและต้องรับการรักษา แต่ทางครอบครัวเด็กปฏิเสธที่จะให้เด็กรับการรักษาในตอนนั้น การทดสอบสมมุติฐานที่แน่ชัดของโรคไธรอยด์ก็คือการส่งตรวจฮอร์โมน TSH, T3, T4 เพื่อตรวจสอบข้อสันนิษฐานให้แน่ชัด แต่โชคไม่ดีเลยที่ผลการตรวจมันไม่เร็วอย่างที่ต้องการ ต้องรออีกตั้ง2-3 วัน ดังนั้นในวันนี้ดิฉันจึงอยากจะนำเรื่องของเด็ก cretin มาให้อ่านกันอีกรอบหนึ่งหลังจากที่เคยนำมาให้อ่านกันบ้างแล้วในวันก่อนๆ
